Chaotic Canvas provides immersive creative experiences including splatter painting, UV blacklight art, glow art experiences, camps, workshops, educational sessions, mobile studio events, and hands-on artistic activities involving paint, tools, lighting effects, and shared creative spaces.

Participants agree to follow all staff instructions, use materials responsibly, respect equipment and fellow participants, avoid unsafe behavior, and comply with all safety guidelines.

I understand participation may involve minor risks including slips, trips, paint exposure, staining of clothing or belongings, accidental spills, physical movement, allergic sensitivities, and general event-related risks. I voluntarily choose to participate.

To the fullest extent permitted by law, I release and hold harmless Chaotic Canvas, its owners, instructors, staff, volunteers, contractors, affiliates, venue hosts, and event partners from claims, damages, losses, expenses, injuries, or liabilities arising from participation in Chaotic Canvas activities.

I understand paints and artistic materials may stain or damage clothing, shoes, accessories, electronics, furniture, vehicles, or personal belongings. Protective gear may be provided but does not guarantee protection from staining or damage.

Activities may involve paint, artistic materials, UV blacklights, colored lighting effects, music, movement, and shared creative spaces. Participants with allergies, medical conditions, or sensory sensitivities should notify staff before participation.

By signing this agreement, I certify that I am the parent or legal guardian of the minor participant and have authority to sign on the minor's behalf. I agree to these terms for myself and the minor participant.

Chaotic Canvas reserves the right to remove participants engaging in unsafe, destructive, threatening, illegal, disruptive, or inappropriate behavior without refund.

Participants may be responsible for damage resulting from intentional misuse of materials, equipment, facilities, or event spaces.

If permission is granted below, I authorize Chaotic Canvas to use photographs, videos, audio recordings, artwork, and participation images for social media, websites, advertising, educational materials, and promotional purposes without compensation. Media permission is voluntary and is not required for participation.

In the event of illness, injury, or a medical emergency, I authorize Chaotic Canvas staff to seek appropriate emergency assistance for myself or my minor child if I cannot be reached immediately.

This agreement shall be governed by the laws of the State of Arizona.
